If you manage the care of another person, such as a child, inform a team member to assist with enrollment.

  • Answer the security question and agree to the terms of use and privacy policy.

HEALTH INFORMATION MANAGEMENT (HIM)

Health Information Management (HIM), also known as Medical Records, refers to the department responsible for managing patients’ medical record data. This includes acquiring, analyzing, and protecting medical information—whether digital, paper-based, or a hybrid format—to ensure quality patient care.

HIM involves collecting patient information for those admitted, treated in the Emergency Department, or receiving outpatient care from a licensed healthcare provider. This information is stored in the Medical Records department. HIM professionals are tasked with ensuring the integrity, security, and accessibility of patient health data. The data can exist in various formats: paper-based, hybrid (a combination of paper and digital), or fully electronic health records (EHR).

HIM professionals collaborate with physicians and other healthcare providers, adhering to HIPAA laws to protect patient privacy. They utilize computer systems for data analysis and coding, which supports timely financial reimbursement for healthcare services rendered.

The HIM department ensures that clinical information is accurate, complete, and secure, enabling healthcare providers to make informed decisions regarding patient treatment plans.

Frequently Asked Questions (FAQ)

1. Who has access to my medical records?

Patient medical records are confidential and protected by legal and regulatory standards. While the information in a medical record belongs to the patient, the records themselves are the property of Rehoboth McKinley Christian Health Care Services. Disclosure is allowed only under specific legal circumstances, such as to government agencies or for de-identified research purposes.

2. How long must medical records be retained?

  • General Records: Retained for at least 10 years after the last treatment date.
  • Radiology Films: Retained for 4 years after exposure; for minors, until the child reaches age 19.
  • Records of Minors: Retained until the patient is 21 years old.
  • Billing Records: Retained for at least 2 years. Healthcare facilities must provide patients with a written copy of their policies on medical record retention. 

3. What is a legal guardian, curator, or personal representative?

If a patient is deceased or unable to provide consent, records may be released to a legal guardian, curator, or personal representative upon presentation of proof of court appointment.

4. Who is considered next of kin?

If the patient is deceased, the personal representative or the next of kin in line of intestate succession may authorize the release of records.

5. What identification is required to access a medical record?

To access medical records, requesting parties must present a valid government-issued photo ID, such as a driver’s license or passport.

6. How long does it take to obtain my medical records?

Medical records must be made available within 30 days of the request or 60 days if stored off-site, with a potential 30-day extension. Rehoboth McKinley Christian Health Care Services strives to fulfill requests within five business days.

7. Can providers charge for copying medical records?

Yes. Providers may charge a reasonable, cost-based fee for copying records, including supplies and labor.

8. How can I obtain a copy of my child’s medical records?

A parent or guardian must provide written authorization unless the minor legally consents to their medical treatment.

9. How do I obtain copies of my lab results?

Lab results can be provided directly to patients as permitted by HIPAA and CLIA regulations. A release form must be completed, and the HIM department can assist—call (505) 863-7265 or fax (505) 863-7214.

10. How do I obtain my radiology report?

After completing a release form, radiology reports are available on a disc (CD). Contact the Radiology Department where the images were performed.

11. How can I correct an error in my medical record?

Under HIPAA, patients can request amendments to their records to add missing or corrected information. However, existing records cannot be removed or altered. Requests must be addressed within 60 days, with a possible 30-day extension.

12. How do I coordinate out-of-state transfers?

To transfer records to an out-of-state provider, submit a written request including your full name, date of birth, discharge date, and your provider’s contact information. There is no fee for transfers to another healthcare provider.

Admissions & Discharge

Admissions

When registering for admission, please have the following information ready:

RMCHCS does not assume responsibility for patients’ valuables. It is recommended that valuables be sent home with a family member. If this is not possible, the RMCHCS Security Department can assist in storing valuables during your hospital stay.

Discharge

After a provider orders a discharge, a nurse provides written instructions for home care. These instructions are reviewed with the patient and their family before leaving the hospital.

The RMCHCS Case Management team is available throughout your hospital stay to discuss your individual health care options for transitioning out of the hospital when you are medically ready for discharge.

Important Steps Before Discharge:

  • Ensure that you understand all instructions regarding your care at home.
  • Address any questions or concerns about your health or discharge plan with your healthcare team.
  • Do not hesitate to ask for clarification if any instructions are unclear.

The RMCHCS Case Management Team is available throughout your hospital stay to discuss healthcare options and assist with your transition out of the hospital when you are ready for discharge.

Medical Directives

Advance Medical Directives

RMCHCS respects patients’ rights to make decisions about their healthcare, which should be documented in their medical records.

Federal law requires hospitals to inform patients that they have the right to accept or refuse medical care and to inform them of the medical consequences of their decisions before and during care delivery.

An Advance Medical Directive is a form that allows you to plan for the care you would want if you could no longer express your wishes. This form lets you specify the medical treatments you would or would not want and designate a person (or persons) to make healthcare decisions on your behalf if you are unable to communicate your preferences. RMCHCS honors Advance Medical Directives for all patients in accordance with applicable laws, regulations, and the hospital’s capabilities.

Durable Power of Attorney for Healthcare

This form allows you to appoint someone as your agent or spokesperson. The agent makes decisions about your treatment only if you are unable to speak for yourself.
You do not need to be at the end of life for this to apply; your agent could speak for you if you are in a coma but are likely to recover.

Instructions for Healthcare

This form allows you to specify the medical care you want if you can no longer express your wishes.
These instructions take effect only when you are unable to communicate your preferences.

Hospital Pricing

The Centers for Medicare and Medicaid Services (CMS), as part of the 2019 IPPS Final Rule, requires hospitals to post a listing of standard charges online.
Rehoboth McKinley Christian Health Care Services supports the intent of these regulations to educate patients about the cost of healthcare and their financial responsibilities. We can provide estimates for services to be performed.
